Submit Your Order...

Our shopping cart does not automatically add shipping. Please use the Add Shipping form to add the appropriate shipping charges.

Please fill out this form completely, including your e-mail address. The requested information will allow us to contact you in the event of problems with your order and promptly mail your items to you.

When your check or money order arrives we will ship your order.

To place a credit card order, fill out the order form to process your order. Then call 972-208-2470 between the hours of 8AM - 8PM CST to supply your card number information.

LABMED now also accepts PayPal.
Click on the button to make your payment, you will then be returned to this page to submit your order.

Once your form is complete, please print a copy for your records and mail a copy along with (if not paying by credit card or PayPal) your check or money order (US Funds Only) payable to LABMED to:
